CSAN Forms Page

   Forms are available below in several formats as noted below each form.  The first format involves printing a form directly using your web browser.  Note that different browsers may produce different results when printing a form in this way.  The second format available is Microsoft Word 97.  The third format is MS Word 2000.  To use either of the last two formats, you must have the appropriate word processing software, i.e. Microsoft Word.

  To use either of the MS Word formats, you will have to right click on the appropriate hyperlink, then left click on the "Save Link As" choice that pops up.  You will be given a window that will allow you to direct the saved file to the folder of your choice on your C: drive or other drive.  Once you have saved the file, you may open it from your C: (or other) drive at your convenience using MS Word.

FORMS:

• USASA Player Registration Form - Used to register a player with CSAN, USASA and USSF for the registration year (which ends on August 30th of each year).

• USASA Player Transfer Form - Used to transfer from one team to another during the registration year (which ends on August 30th of each year).

• Player Liability Insurance Waiver Form (team version) - each player on an affiliated team must sign a line of this form in order to comply with the terms of the USASA liability insurance policy.

• Player Liability Insurance Waiver Form (single player version) - use this waiver form when you sign a single new player after having turned in the team version of this form.

• Parental Permission and Waiver Form - players under the age of 18 who play for a CSAN affiliated team must sign this form and also obtain the signature of at least one parent.  (Please note the places where this word processing file has been customized for a specific league.  Substitute your own league's name in the appropriate places before you print this file.)

• Team's Annual Mandatory Form - each affiliated team must submit this form to the CSAN office once a year prior to the start of their first playing season.  Valid and completed player passes will only be sent to teams that have submitted a properly completed annual mandatory form.

• Form to Request a Certificate of Insurance - you may need to show proof of liability insurance in order to obtain you field use permits.  Use this form to request such proof from the CSAN office in San Francisco.
