PERSONAL ACCIDENT INSURANCE
CARRIER CHANGED
Included with every CSAN player's registration is
a personal accident insurance (PAI) policy. USASA recently changed
the company that carries it personal accident insurance.
The new company has a different set of claim forms, so any
claims stemming from accidents occuring after September 1, 2011
must use the new forms. |

Directors and Officers Insurance is Free to Affiliated Leagues
Leagues affiliated with California Soccer
Association North may obtain free Directors and Officers
Insurance simply by filling out a one page form and submitting
it to the CSAN office. Call the CSAN office toll free at
(866) 404-2726 to obtain a copy of this form. |

2012 CSAN Hall of Fame
Dinner
The CSAN Hall of Fame Dinner is
not that far away (August, 20121). It's time to start
thinking about nominating someone worthy from your league who
has contributed to the game on multiple levels.
Nominations are due no later than December 31, 2011.
